For your convenience, we offer several options for submitting claims. Be sure to include all required documents for your state to ensure the claim can be processed quickly.

Electronic deposit
Another convenient option is to receive claim payments via direct deposit. To register, fax the completed Automatic Claim Deposit form and a voided check to (800) 320-3291.
If you are set up for Automatic Claim Deposit, you will receive an email confirmation when your claim has been paid. If you are not registered for Automatic Claim Deposit, contact licensing@gafg.com; so that you can receive the most up-to-date information.
Your claims will be mailed to the address on your claim form if an ACH form is not submitted.
